健康 Requirement Checklists

Many programs require a fieldwork experience (e.g. clinical, internship, practicum, etc.) as part of the course of study. Find your program-specific requirement checklist below. It lists everything you need to submit with resources conveniently hyperlinked.

健康 requirement files must be submitted online using the appropriate Dynamic Form link below.

Name and date of birth required on all attached files-handwritten is acceptable. 需要学生证. CORI必须经过公证. Find a Notary Public at your bank or town office.

Once your files are reviewed an email notice will be sent listing remaining items due and/or subsequent action(s) required. 请监控垃圾邮件.

Vaccination records can be accessed using the Massachusetts Immunization 信息 System 或者是 Connecticut Department of Public 健康 Portal.

If COVID-19 vaccination is on your checklist, you may request accommodation by submitting the Request for Reasonable Accommodations found on our COVID-19 常见问题 page.
